Home Delivered Meals:

These meals are delivered to seniors that are homebound, and unable to cook, or shop for themselves. There are two kinds of meals available - hot and frozen. The hot meals have temperature controls that are state mandated, so the delivery routes are limited. To fill the gap that distance limits frozen meals were added that will meet nutritional requirements the Federal Government mandates for seniors. The 5- pack of frozen meals can be picked up once per week at a local center, or can be delivered. The frozen meals can then be reheated and will fulfill 1/3 of daily requirements.


Eligibility:

Any homebound Senior (60 years of age or older) who are unable to prepare a balanced meal themselves or have anyone to prepare a meal for them is eligible for the nutritionally balanced meals.

 

CCSP - Home Delivered Meals

Community Care Service Program (CCSP). This is a Medicaid waivered program that provides home and community based services. CCSP is an alternative to nursing home placement, and is much cheaper than Medicaid nursing home costs.
The “Area Agency on the Ageing”, (AAA) handles the referrals for CCSP, and for our other programs.
