HOMEMAKER PROGRAM
Seniors who are unable to perform fundamental housekeeping tasks, but are otherwise capable of living independently are eligible for this service. A homemaker is a trained and supervised person who provides assistance to older persons in the performance of home-related activities. This service helps make it possible for seniors to remain independent and in their homes safely.
The Older Americans Act, funds the Homemaker Program, and a referral package is sent from the Area Agency on Aging. A Homemaker supervisor then conducts an interview and assessment with the client. Potential clients must be sixty years of age or older, and in the following situation.
Criteria to be eligible for Home Maker Services:
Potential client who lives with family members and are doing all they can do to keep a Senior living at home, but need additional assistance.
The caregiver is out of the home or temporarily incapacitated in the home for a limited period and a Homemaker is needed to provide assistance.
Care Plan:
Before the service can be rendered, a written individualized care plan must be developed that identifies the service components based on the client’s needs. The client must participate in the development of the Care Plan, and may include authorized representatives, or family members. The plan may be modified to reflect any change in the client's needs, and each plan must include the following.
1. An assessment of needed services
2. Services to be delivered by the service provider.
3. Goals and objectives of service to be provided.
4. The quantity of service units to be provided.
Service Agreement:
A service agreement will then be completed between the client and the service provider. The agreement will explain the service arrangement to the client. The client shall receive a copy of the agreement.
The agreement shall include:
Services to be provided.
Scheduled hours and days of service.
Information regarding voluntary contributions.
Emergency contacts.
